The undersigned agrees to the following conditions of use: That this Permit shall be presented to the supervisor in charge. Play-ability fields shall be determined by the maintenance personnel, the park supervisor or designated City of East Orange representative in authority. This decision shall be final. The obtainee of this Permit is responsible for the conduct of all persons participating in the above event, including spectators.

I, the undersigned, have read the foregoing APPLICATION and I agree to abide by all ordinances pertaining to the use of public recreation facilities. NOTE: PERMIT IS FOR NORMAL USE OF THE PARK OF THE RECREATION & CULTURAL AFFAIRS DEPARTMENT. If any unusual programs are to take place, in many instances special permission or permits are needed. If you do not know whether your activity requires a permit, you may contact our office at 973-414-4141. NO SALES OF ANY TYPE ARE AL-LOWED IN THE PARK. I understand that the City will not be held liable or enjoined in any liable suit against the City that might endanger the health, welfare and safety of its citizens if above special permits are secured. All Recreation Activities supersede any permit approval. Permit maybe revoked for not comply-ing with department policy. You are required to clean the park after your event.

HOLD HARMLESS AGREEMENT 1. The applicant agrees and understands that the City of East Orange carries insurance covering its legal liability. City of East Orange assumes no liability of the Applicant or of the participants or invitees of the applicant. The applicant further agrees that City of East Orange assumes no responsibility for damages and or theft of property that may arise out of permitted use of its facility. 2. The applicant agrees to indemnify City of East Orange and save it harmless from and against any and all claims, actions, damages, liability and expense which includes but not limited attorney fees in connection with loss of life, personal injury and / or damage to property arising from or out the occupancy or use by the applicant of City of East Orange facility occasioned wholly or in part by any act of omission applicant, its agents, contractors, subcontractors, employees, representatives or invitees.
